Add Sections and Fields

After creating a monitoring template, the next step is to organize it into sections and add the monitoring fields that field officers will complete during visits.

Sections help group related questions together, making monitoring forms easier to complete and review.


Open the Monitoring Template

  1. Navigate to MonitoringMonitoring Templates.
  2. Open the monitoring template you want to edit.

From here, you can create sections and add monitoring fields.


Create a Section

Within the monitoring template, click Add Section.

Section Name

Enter a descriptive name for the section.

Examples include:

  • General Information
  • Infrastructure
  • Service Delivery
  • Staffing
  • Compliance
  • Observations
  • Recommendations

After saving the section, it becomes available for monitoring fields.


Add a Monitoring Field

Within the desired section, click Add Field.

Each monitoring field represents a single question or data point collected during a visit.


Field Label

Enter the question or label that will be displayed to field officers.

Examples:

  • Visit Date
  • Facility Name
  • Number of Staff
  • Is PPE Available?
  • Overall Condition
  • Comments

Use clear, concise wording that is easy to understand in the field.


Suggested Fields

FieldOps includes a Suggested Fields list containing commonly used monitoring questions.

Selecting a suggested field automatically populates the field label, helping you build templates more quickly while maintaining consistency across projects.

You can also create completely custom fields if the suggested list does not meet your requirements.


Field Type

Choose how the response should be collected.

Available field types include:

  • Text
  • Number
  • Textarea
  • Select (Dropdown)
  • Radio Buttons
  • Multi Select
  • Checkbox List
  • Checkbox (Yes/No)
  • Date

Each field type is designed for different kinds of data collection.


Required Field

Use the Required toggle to determine whether the field must be completed before a visit can be submitted.

When enabled:

  • The field is marked as required.
  • Field officers must provide a value.
  • FieldOps validates the response before allowing submission.

When disabled:

  • The field becomes optional.
  • Visits can be submitted without completing the field.

Building Your Template

Continue adding sections and monitoring fields until your template reflects the information you want field officers to collect.

A completed template might look like this:

Best Practices

When designing monitoring templates:

  • Group related questions into logical sections.
  • Keep section names short and descriptive.
  • Use clear, unambiguous field labels.
  • Mark only essential questions as required.
  • Reuse suggested fields where appropriate.
  • Review the template before using it for field visits.
