Create a Monitoring Template¶
Monitoring templates define the structure of the forms used during field visits. Before field officers can collect monitoring data, you must create at least one monitoring template.
A monitoring template contains one or more sections, and each section contains the monitoring fields that users complete during a visit.
Before You Begin¶
Before creating a monitoring template, ensure that:
- Your workspace has been set up.
- You have permission to manage monitoring templates.
Note
Monitoring templates can be reused across multiple programmes, projects and visits.
Creating a Monitoring Template¶
- From the left navigation menu, open Monitoring.
- Select Monitoring Templates.
- Click New Monitoring Template.
You will be asked to provide the template details.
Template Details¶
Template Name¶
Enter a descriptive name for the monitoring template.
Examples:
- Health Facility Supervision
- School Inspection Checklist
- Water Point Assessment
- Quarterly Project Monitoring
Choose a name that clearly identifies the purpose of the template.
Description¶
Provide a brief description of what the template is used for.
This helps administrators identify the correct template when multiple templates exist.
Example:
Quarterly monitoring checklist used during health facility supervision visits.
Save the Template¶
After entering the required information, click Create.
The monitoring template will be created and is now ready for sections and monitoring fields.
At this stage, the template does not yet contain any questions.
What Happens Next?¶
After creating a template, you can begin organizing your monitoring form by adding sections.
Typical sections include:
- General Information
- Infrastructure
- Service Delivery
- Staffing
- Compliance
- Observations
- Recommendations
Each section can contain one or more monitoring fields.
Best Practices¶
When designing monitoring templates:
- Use clear and descriptive template names.
- Organize questions into logical sections.
- Keep related questions together.
- Reuse templates whenever possible instead of creating duplicates.
- Review the template before using it for field visits.
